Good Safety Engineering

Us in the oil and gas industry always say that safety is paramount. We have catchphrases to impress this into the work culture:

  • ‘No one gets hurts’
  • ‘Nothing is so important that it cannot be done safely.’

Engineers work hard to ensure that there are design safeguards in place:

  • Perform specific reviews to identify and minimise safety hazards (HAZOP, HAZID, QRA)
  • Hire a safety engineer to champion safety causes.
  • Physically restrict flows such that downstream systems cannot be overpressurized (restriction orifices)

If a risk can’t be eliminated by design, then procedures are put in place and enforced to reduce the risk to the people and assets:

  • Work Permit Systems.
  • Safety Training.
  • Lock Out Tag Out (LOTO).

The photo below shows a design feature I like that has been incorporated into a water cooler. Note the knob on the hot water dispenser button. The knob has to be rotated before you can press the button. This reduces the risk that a child could scald themselves if they get it in their minds to experiment with the unit.

Why not put the knob on all the buttons, you may ask? Well, that is the realm of risk analysis, and cost-benefit studies. The convenience of dispensing tepid or cold water outweighs the risk of a messy puddle cause by a young experimentalist.

Another thing this brings to mind is an analysis of applicable features. Would you require this (presumably more expensive) feature if the water dispenser is in an office environment?

Saturday Star 2008-10-11 – Job Opportunities

Another week, another scan of the Saturday Star newspaper. Here’s a list of job ads in the paper:

  • PETRONAS (all capitals, please) is recruiting. The online ad version of The Star isn’t clear, but I think there’s an Open Day from the 24th-25th Oct, 2008 at the JW Marriot, KL. I wonder why they didn’t do it next door at the Mandarin. Too expensive?

It must still be a slow week, that’s the only engineering job I can find.

I was curious as to what online engineering forums Malaysian engineers use to seek information. Okay, I am assuming you have Internet access in your office. First off, I hope that you use my blog and topics to start discussion threads (and maybe I make a buck or two if you find useful stuff).

News – Released MISC vessels being escorted to Djibouti

From the Star:

KUALA LUMPUR: The two hijacked MISC chemical tankers MT Bunga Melati Dua and MT Bunga Melati Lima are being escorted to Djibouti by the Malaysian Navy following their release by Somali pirates on Saturday and Monday, respectively.

Petrolium Nasional Bhd (PETRONAS) president and chief executive officer Tan Sri Mohd Hassan Merican said the vessels were still within the danger zone, and that it would take a few days for them to reach Djibouti.

“All crew members aboard both vessels are safe and relatively healthy. They are traumatised, but are being attended to by our trauma team,” he told a press conference at the MISC office here yesterday.

He admitted that a ransom was paid, but refused to say how much. Media reports claimed the firm had paid US$2mil (RM6.9mil) ransom for the release of each vessel.

He said although paying ransom was against the company’s policy, MISC had to pay to secure the release of its crew members.

“We delivered the money to them with the help of our navy, it was cash on delivery.”

Doesn’t the second to last sentence above seem contradictory, and an opening for anyone to traumatize PETRONAS company staff for the monetary rewards?


Petrol Price Drop (again) Malaysia

KUALA LUMPUR, Sept 24 – Malaysia will cut petrol prices by 10 sen to 2.45 ringgit ($0.719) per litre from Thursday, Domestic Trade Minister Shahrir Samad said on Wednesday.

The government also cut diesel prices by 10 sen to 2.40 ringgit a litre, Shahrir told reporters.

This the second reduction in fuel prices in a month following lower global crude prices. The move is likely to help curb inflation, which hit a 27-year high of 8.5 percent in July and remained at that level in August, according to data released on Wednesday. ($1=3.408 Malaysian Ringgit)

 For those in Malaysia: you must know this by now. Those overseas: this is an interesting titbit of information.
