Oil refinery ablaze in Port Dickson

A fire broke out at a Petron oil refinery at Batu 1 here tonight, temporarily disrupting its operations. The fire, which began at 9.30pm, saw 10-metre-high flames shoot up, believed to be due to leaking on the refinery stacks. More than 30 firemen and seven fire engines from the Port Dickson, Teluk Kemang, Seremban and Seremban 2 fire stations were dispatched to the scene. The firemen took about an hour to put out the blaze. A witness, M. Suresh, 60, said he heard two loud booming sounds before the fire broke out. “We were shocked and worried as our houses are only 300 metres away from the location,” he said. No injuries have been reported at press time.

 


Helicopter tragedies worrying, says deputy minister

Helicopter operations need to be scrutinized in order to avoid any disaster or tragedy in the future, said Deputy Transport Minister Aziz Kaprawi.

He said the number of helicopter tragedies in the country was a bit worrying.

“Probably (the helicopter operations) need to be more refined and include the pilots. Definitely (relevant parties) need to identify the weaknesses in helicopter operations,” he told reporters after breaking fast with media personnel here last night.

Asked on the latest development on the helicopter crash in Sebuyau, Sarawak, on May 5, Aziz said the main frame of the helicopter was still missing.

The search would continue, he added.

 


Oil and gas giant awards scholarships to Malaysian UQ health and safety students

PETRONAS, the Malaysian oil and gas giant has recently awarded scholarships to four Malaysian students studying occupational health and safety science at the University of Queensland.

The students, Nurul Jasmine Shaifful Anuar, Crystal Lwi, Ying Huei Ho and Huey Fang Woo have been awarded the prestigious academic scholarships.

The University of Queensland is recognised as one of the world’s top 100 ranked universities. The university’s Bachelor of Occupational Health and Safety Science (Honours) is unique in Australia.

The idea is that a cause or hazard leads to a consequence only if all the layers fail. This is visually shown in a Swiss cheese diagram:

This is the Cheesecake diagram:

Though, some items are the same:

  1. There are layers
  2. If one layer is defective, it may not affect the overall facility
  3. If all layers are defective, go get your money back.
  4. Don’t do technical safety while hungry.

Gas leak explosion kills Petronas worker, injures three others

A worker at a gas processing facility in Malaysia operated by an affiliate of Petronas was killed on Thursday after an explosion.

Petronas Gas (PGB) confirmed that the employee “succumbed to his injury while doing repair work” at near Gas Processing Plant 4.

The processing facility is located in Kertih, Terengganu, Malaysia.

“PGB is extending all necessary assistance to the family members of the victim,” the company said.

According to local media, the deceased worker was a welder.

Three others were injured in the explosion.

The four workers were performing maintenance to repair a gas leak.
