Safety vs. Profits – Civilian Life

In the oil and gas industry, we claim that safety is paramount, more important than profits or cost. An oil and gas engineer will:

  • Design a fail safe asset.
  • For existing assets, additional safety equipment may be installed if the standards of safety have changed, or as low as reasonable practicable (ALARP) requirements have changed.
  • If hardware can’t be designed or installed, we introduce procedures that staff can follow to minimise risk. Not the best solution, but a solution none the less.

In the real world, safety might have a different priority. I took the following photos at Cosmo’s World Theme Park, Berjaya Time Square, Malaysia. Emergency doors are installed, in case a quick emergency exit is required. However, I guess that they have had problems in the past, where ‘customers’ come in through the door and bypass the paying facilities. You gotta wonder, how exactly does this door function?

To their credit, if you did have to exit via this door, you find this little item next to it, kinda like the old style adventure games:

I for one probably won’t find the key in a real emergency. I wonder if their fire drills are witness and approved by Bomba.

Keeping a Tidy Fab Yard

During my visit to a neighbouring country, I met up with an old acquaintance of mine. He told a tale of the fabrication yard he was in charge of. The yard was under the receivership or supervision of a bank, such that all the assets of the yard were owned by said bank. It got so bad that the yard staff were afraid to carry out housekeeping by disposing of scrap metal and junk equipment.

Scrap is scrap. It’s worthless as it rots in your yard, could be sold for money, and it occupies valuable space. It can cause a safety hazard, both physical (it’s sharp and could have a tendency to collapse according to Sod’s law), biological (mosquitoes, potential injury points), and procedural (moving stuff around a full fab yard is not a fun activity).

In the end, my friend got the bank to agree to disposal of the scrap, and doing some honest to goodness housekeeping.

Reminded – Not all Accidents Happen at Site

While driving near my house, I saw an accident where concrete struts had fallen off the transporting truck. This brings home that accidents and incidents don’t only happen at site, but can happen during the commute to and from the site. Think of it, equipment is being transported by third parties, who claim that they meet your safety expectations, through civilian traffic which definitely doesn’t meet your safety expectations here in Malaysia.

If I was doing a risk assessment and having to fill this out on a risk matrix, the consequence of getting bad publicity (do you still want your logo on the equipment being transported?) is high, compared with an accident happening in the remote reaches of Kalimantan, or just having  you stuff sink to the bottom of the South China Sea.

Of course, imagine if the cement had dropped onto an occupied vehicle which happened to be next to the truck at the time of the incident.
