Malaysia’s Petronas Gas plant catches fire, one dead

Dateline 2012-05-10:

 A gas processing plant of Petronas Gas Bhd in the north-eastern Malaysian state of Terengganu caught fire on Thursday afternoon, causing one fatality with two others hospitalised, the company said.

The natural gas processing arm of state oil firm Petroliam Nasional Bhd (Petronas) said employees of Hyundai-PFCE Consortium (HPC), which is the contractor servicing the gas processing plant during shutdown maintenance, were affected.

“The remaining affected personnel have received appropriate medical treatment,” Petronas Gas said in a statement late on Thursday, without disclosing the exact number of employees affected by the fire.


24 Petronas workers hurt in gas leak

Dateline 2012-04-07:

 24 workers at the Petronas gas processing plant in Kertih were admitted to hospital with breathing difficulties caused by a gas leak on Thursday.

Around 300 workers were about to start maintenance work shortly after 8pm when they had to rush out of the plant after smelling the gas, believed to be ammonia.

Eight of the workers were sent to Dungun Hospital while 16 others received treatment at the Paka clinic.

Man killed in NGV explosion

MALACCA: Two brothers who converted a cooking gas tank into a natural gas kit for their van paid dearly when it exploded, killing one of them.

Mohd Hailmi Ishak, 25, from Kampung Duyong, and his brother Rosli Ishak, 30, from Taman Tiang Dua, had stopped at a PETRONAS petrol station in Batu Berendam at about 7.50pm on Friday to refuel.

Their worker Razali Md Zin, 31, was with them.

It is believed that Rosli, the driver, was refilling the van at the natural gas vehicle (NGV) stand while his younger brother was standing nearby when the blast happened.

The explosion shattered the front portion and two doors of the vehicle.

It shattered Mohd Hailmi’s left leg and ripped open his abdomen while Rosli and Razali escaped with light injuries.

You see a lot of this jury rigging work offshore, difference is, the jury rigging is engineered as opposed to ‘try and see lah’ philosophy. Thing’s I’ve done are:

  • Use a flange that had a higher than required rating.
  • Use a smaller than design size valve (it’s okay, the process flow had been reduced).
  • Use a process compressor for other than it’s intended use (don’t worry, risk analysis was performed).
  • Route liquid to different vessels (there’s a whole story behind this one).

This is where your engineering (and presentation and persuation) skills are called to the forefront.


Deadly Industrial Area – Kertih

I was working at the Onshore Slug Catcher (OSC) yesterday, performing tasks related to custody measurement. At about 3 o’clock, word came down that there was a accident in front of the Terengganu Crude Oil Terminal (TCOT). From secondhand information, it seems that a Proton was turning right into the TCOT offroad to perform a U-turn. A Sorento attempted to overtake the Proton at the same moment it was turning in.

The above scenario ended up with the Sorento taking a barrel roll, producing 6 victims. I saw the cars being towed in the direction of Paka at 7 pm. The Proton looked as it had been side swipped, whereas the Sorento had one side crushed in.

The Kawasan Industri Petroliam PETRONAS (KIPP) in front of the TCOT is undergoing extensive roadwork. The traffic flow also changes frequently, as they upgrade the road from two to four lanes. Traffic is fast, taking advantage of the already upgraded section. All this adds up to a dangerous and deadly situation.

I hope that authorities (not sure whether that is PETRONAS or the local Terengganu council) improve the situation, providing better traffic control, and finish the road upgrade soon.
